 The Taunton Deane Symphony Service.


Symphony is a new service across Taunton Deane (also in some other parts of Somerset) to provide a new type of service for people with 3or more long term (or chronic) conditions.

If you have 3 or more long term conditions, the service aims to support you to understand more:

  • about your conditions generally
  • how these conditions impact on your life and how you can reduce this impact
  • how to manage your health to remain well
  • to plan what you might do if you become unwell – either with an acute illness or if your chronic condition worsens

What is a Long Term Condition (LTC)?

Long term conditions are defined as on-going conditions that will not be resolved but can be managed by medication and other therapies. For Further information on Long term conditions click here


The 8 long term conditions included in the Symphony service are:


Coronary Heart Disease (CHD)

Chronic Kidney Disease (CKD)                          

Chronic Obstructive Pulmonary Disease (COPD)





You will have been diagnosed with these conditions by your GP and your GP practice will already be providing you with care for these.

What is the problem with having long term conditions?

Depending on the severity of your conditions, they may impact not just on your health but on your general well-being – for example managing pain or feeling unwell for long periods can be stressful. You may also be at higher risk of acute illnesses and research shows that people with multiple long term conditions are at higher risk of requiring unplanned urgent or emergency care.

What does Symphony do?

It is recognised that people who understand more about their conditions and who are supported to manage their health feel more in control and can remain healthier for longer.
